  3. Package is applicable to non-complicated cases only. 
  4. Package price excludes any complication-related charges. 
  5. Package offers 2 and 4-bedded room types only. Room upgrade is subject to availability and additional charges to patients. 
  6. Package does not include critical ward admission such as ICU/CCU/HDU. 
  7. Package does not include any other additional investigations, medications, procedures, surgeries which are non-relevant to LSG. 
  8. Package excludes any additional charges incurred exceeding the 3 days 2 nights ward admission.
  9. Package excludes post-operation outpatient clinic follow-up consultation(s). 
  10. Full payment of package price required upon admission. 
  11. A surcharge is applicable on Sundays and Public Holidays observed in Malaysia. 
  12. Package is applicable for self-payment only. Not applicable for insurance claims or insurance pay & claim purposes. 
  13. 0% instalment payment scheme via Easy Payment Plan (EPP) applicable for Maybank, HSBC Bank and Public Bank only. 
  14. Package validity until 30th September 2026.
  15. Package is subject to 6% Sales & Service Tax (SST) for non-Malaysians.
